| Appointments: To Schedule
an appointment, please call 517.349.0027. New Patients are
always welcome. Our receptionists will be glad to assist with
scheduling a time that is convenient for you.
Insurance: We participate
with most insurance companies. If your insurance company is
not listed below, please contact our office at 517.349.0027.
BCBSM, PHP, Blue Care Network, Aetna, United HealthCare, Medicare,
Medicaid, PPOM, SPHN.
Referrals Prior Authorizations:
Some insurance carriers require prior authorization from your
primary care physician. Failure to obtain authorization will
result in no coverage or reduced coverage for services. Please
make sure your prior authorization is in place before your
appointment.
Cancellations: If you are
unable to keep an appointment, we ask that you kindly provide
at least 24 hours notice. This courtesy, on your part, will
make it possible to give the appointment to another patient
needing care.
Financial Policies: You
will be responsible for any co-pay or deductible as determined
by you insurance carrier at the time of service. It is the
responsibility of the patient to know the terms of their coverage.
We will assist to the best of our ability to answer coverage
questions.
Because of the ever changing status of families,
such as divorce, separation and single parents, the parent/guardian
who requests the treatment for the child is responsible for
the charges.
Our fees are comparable to those charged
by other specialists in this area. We will be happy to discuss
all charges for our services at any time and we will gladly
answer all your questions. Patients receive monthly statements
showing transactions when there is a balance due. We will
assist patients with payment plans when requested.
Emergencies: If you require
care after our normal office hours, please call our answering
service at 517.349.0027 to reach the on-call allergist. If
you cannot wait for a call back, dial 911 or proceed to the
nearest emergency room. |
